Social: Client lives with his wife and no pets in a 2 story home with an elevator. The schedule is, Thursdays 7 pm to 7 am. Saturday and Sunday 8 am to 4 pm.
Job Responsibilities:
Diagnoses: Parkinson's Disease with imbalance issues, Mild memory loss
ADL's:
Personal Hygiene: Assist with showering and toileting. Incontinence Issues.
Dressing: Stand by assit with getting dressed. Chooses his own clothes, including socks and shoes
Eating: Feeds himself independently, but needs food planned, prepared and served according to family menu. Must eat when hungry. Drinks independently, but needs reminders and assistance to stay hydrated. Offer snacks if wanted. No swallowing issues at this time. Caregiver will be going by a menu already prepared by family for awhile.
Transferring: Independently mobile with a walker or cane. Standby assist only and observe all transfers. Can get himself up and down out of a chair or reclining position. Home is equipped with an elevator.
**Exercising: Client attends boxing classes twice per week for an hour on Tuesdays and Fridays, in which the caregiver needs to drive him in her own vehicle.
Emotional Health: Client is accustomed to his wife being his primary caregiver.
Qualifications:
Minimum of 1 year experience as an in-home Caregiver, CNA or related experiences with a loved one of family member
Ability to effectively communicate through written and verbal language
Valid Driver's License and adequate auto insurance
Consent to criminal, elder abuse, motor vehicle and sex offender background checks
Consent to drug and alcohol testing
