Lead Job Coach – Vocational Services


Lead Job Coaches will meet the following requirements (with or without reasonable accommodations) for employment with Home Life. This position is eligible for overtime and considered “non-exempt.”


Minimum Qualifications:

  • Six months of experience as a job coach
  • Ability to pass an initial criminal history check from the Department of Human Services Criminal Records Unit and recheck every two years after.
  • Be a citizen of the United States or legally able to work in the United States
  • Eighteen years old and high school graduate or equivalent.
  • Current driver license, insurance and a good driving record.
  • Must be willing and able to transport supported individuals in personal vehicle with company approved reimbursement rate per mile.
  • Ability to provide services designed to support successful employment outcomes consistent with individualized career goals, including goals identified in an individual's Individual Support Plan (ISP) and Career Development Plan (CDP).
  • Ability to support individuals to maintain and be successful in employment.
  • Demonstrate by background, education, references, skills, and abilities, the employment professional is capable of safely and adequately performing the tasks to support an individual's Service Agreement or ISP and CDP including, but not limited to, all of the following:

o Ability and sufficient education to follow oral and written instructions and keep any records required.

o Responsibility, maturity, and reputable character exercising sound judgment.

o Ability to communicate with the supported individual.

o Training of a nature and type sufficient to ensure the employment professional has knowledge of emergency procedures specific to the individual receiving services.


  • Complete a Department-approved competency-based Supported Employment training within 90 calendar days from the date of employment.
  • Complete the Department-approved online core competencies for supported employment professionals within one year from the date of employment.
  • Complete twelve continuing education credits (CECs) of Department-Approved training on an annual basis. At least six of the twelve CECs required must be specific to the topic of Supported Employment as it relates to people with developmental disabilities. The remaining CECs for the year, if any, must be Department-approved trainings related to topics about disability and not necessarily about Supported Employment.
  • Complete a Department-approved training for Discovery prior to delivering Discovery services.
  • Ability to read and understand written and verbal orders.
  • Ability to perform basic computer operating skills.
  • Regular attendance and ability to perform tasks outlined in this description (with or without a reasonable accommodation) is an essential function of this position.

Preferred Qualifications:

  • Associates Degree in Human Services or related field.
  • 1 year of experience working as a job coach or otherwise related role
  • Experience working with and/or supporting individuals in supported employment or vocational related work.
  • Knowledge of the Vocational Rehabilitation and Developmental Disability Supported Employment systems.

Essential Physical Requirements:

  • The physical effort typically required in this job includes, but is not limited to, be on feet for extended time, work outdoors and indoors, keying/typing, pushing, pulling. There may be lifting up to 110 lbs., transferring or repositioning supported individual assistance up to 250 lbs, operating a wheelchair van and lift, and behavior supports up to applying physical holds (as per Oregon Intervention System OIS instruction) and other duties as needed with or without reasonable accommodation.

Responsibilities

General Expectations:

  • The Home Life Vocational Lead Job Coach is responsible for supporting individuals in pre and post-employment skills, working to ensure that the individual is being supported in Discovery, Employment Path, and/or Job Coaching as assigned.
  • The Lead Job Coach reports directly to the Employment Program Manager (EPM), while in collaboration with the Vocational Services Coordinator (VSC), and Job Developer(s).
  • Work as a team with the other Job Coaches in Home Life’s Vocational Program to ensure the best quality of services are provided.
  • Ensure that working hours are kept correct as outlined for your shift in your specific program or as directed by the Program Manager.
  • Assure that all pertinent information regarding supported individuals, major issues, or the program is documented in Therap.
  • Assure that all program and supported individuals’ needs are met.
  • Assure that all program supplies/equipment are available and in good working order.
  • Adhere to all policies, rules, and procedures of Home Life and the Vocational Program.
  • Attend and participate in staff meetings unless excused by the Employment Program Manager (EPM).
  • Attend all competency based training as assigned by the EPM, VSC, or Training Coordinator.
  • Complete and hand in all reports, timesheets, and tracking forms by their due dates.
  • Assume additional responsibilities as assigned/negotiated/needed by the EPM or VSC.
  • Complete all required paperwork in a timely and professional manner.
  • Maintain itemized receipts for all reimbursements with petty cash.
  • Routinely check When To Work for schedule and updates.
  • Attend all scheduled shifts or find coverage in an appropriate time.

Health and Safety of Clients:

  • Assure that GER’s are completed according to instructions provided.
  • Conduct fire drills as assigned by the EPM.
  • Assure that fire drill records are documented according to instructions provided.
  • Assure that EPM is made aware of any problems, which arise on evacuation, e.g. failure to evacuate within a reasonable amount of time.
  • Assure that supported individuals’ confidentiality is maintained.
  • Respond appropriately to the posted Emergency Preparedness Plan.
  • Follow all safety plans and protocols.


Support Services:

  • Support individuals with on-the-job training and communication as needed.
  • Support all individuals with job readiness and employment path skills.
  • Assure that supported individual’s schedules are followed and that supported individuals are present for scheduled activities.
  • Complete assigned core-competencies within the outlined time frame.
  • Coordinate, write, and submit Discovery profiles as assigned. Submit the Discovery to the EPM, VSC, or Job Developer for review and edits before submitting to the individuals’ team.
  • Assist with writing and submitting VR Job Coaching documentation including the Job Coaching Plan and Reports.
  • Read all pertinent Therap entries on a regular basis.
  • Understand that all agency and supported individual coordination of information (ex: parents, residential, County, etc.) will be done by the EPM unless otherwise assigned.
  • Assure that all supported individuals’ employment supports are being met as outlined in the individual’s ISP and they are making progress towards goals.
  • For all individuals in Job Coaching Services, adhere to the following:

o Obtain supported individuals’ work schedule and changes in timely manner and share with the EPM to provide appropriate scheduling.

o Fill out Employer Info Sheet with appropriate info on how to gather clock ins, paystubs, etc.

o Obtain supported individuals clock in/out times for previous non-coached shifts at each check in, input in appropriate modules in Therapy or scan in and email to EPM.

o Be involved in Person Centered Information collection and Action Planning for ISP meeting

o Actively work toward supported individual’s goals on their action plans and document accordingly.

o Attend ISP meeting if schedule permits and assigned.

o Develop relationship with employer and share contact info with Home Life email.

o Collect relevant information for supported individual employment sheet.

o Check in at least monthly with supervisors at Job-Coaching locations and document comments in a person-centered way.

o Document and share fade plans with the employment team.

o Track type of job coaching techniques and notable info to incorporate into monthly reporting.

  • For individuals in Employment Path Services adhere to the following:

o Develop a plan for the week and month and share with management team.

o Document supported individual’s goals, successes, and support needs.

o Complete a site assessment or other functional assessment with each Employment Path participant at least every 45 days.

  • For individuals in Discovery Services adhere to the following:

o Schedule, plan, and coordinate Discovery activities in line with the requirements independently and efficiently, using the outlined time in When To Work.

o Communicate with EPM or VSC immediately for any changes, challenges, or problem solving.

o Track and record all Discovery activities and hours, including refusals.

o Complete the Discovery on the approved ODDS Discovery Profile and meet requirements as outlined in the Discovery Policy.

o Complete, document, and review the Discovery within 90 days from approval.

Quality Assurance (Lead)

  • Assist management in checking for complete and accurate documentation in Therap.
  • Assist management to complete and send the monthly brokerage reports.
  • Assist with initial on-the-job training for a new position and periodically check in with job seekers and employers to ensure their needs are met.
  • Develop and execute new activities, assessments, and training to meet the service needs.
  • Assist with billing data entry as a backup and be able to audit months of data as needed.
  • Ensure that all employees we support in community jobs are being supported at least the minimum, and as needed, provide check-ins for their progress at least every 6 months, and ideally quarterly.
  • Assist developer and coaches in filling out Vocational Rehabilitation paperwork surrounding job coaching such as Job Coach plans and Job Coaching Reports.
  • Assist coaches with problem solving around services provided.

Other Duties as Assigned:

  • Assume additional responsibilities as assigned by House or EPM, VSC, or Services Director.
