Moorhead Rehabilitation and Care Center is a 60+ bed facility in Moorhead, MN on the Fargo, ND border. Our Moorhead Rehab team is passionate about providing exceptional patient care. Our team is seeking a qualified, dedicated and compassionate Social Services Director. The primary purpose for your job position is to assist in planning, developing, organizing, implementing, evaluating, and directing our facility's social service programs in accordance with current existing federal, state, and local standards, as well as our established policies and procedures, to assure that the medically related emotional and social needs of the resident are met/maintained on a individual basis.

Responsibilities:

• Assist in the planning, developing, organizing, implementing, evaluating and directing of the social service programs of the facility.
• Attend meetings with administration, medical and nursing staff as directed.
• Assisting and reviewing in development, administering and coordinating of department policies and procedures.
• Participate in community planning related to the interests of the facility and the services and needs of the resident and family.
• Participate in discharge planning, development and implementation of social care plans and resident assessments.
• Refer resident/family to appropriate social service agencies when the facility does not provide the services or needs of the resident.
• Maintain a quality working relationship with the medical profession and other health related facilities and organizations.

Qualifications:

• Outstanding problem solving and interpersonal skills
• Computer experience is required
• Excellent oral/written skills
