Bear Mountain Healthcare is seeking a Full Time (40hr, day shift) Director of Social Services for our skilled nursing facility, Country Gardens, in Swansea, Ma.

Position: The Director of Social Services is responsible for initiating and maintaining a comprehensive social services program on throughout the center. Social Services is responsible for identifying and providing for each resident’s social, emotional and psychological needs. The Director will assist in the development of a safe discharge plan. This position also requires the timely completion of MDS assessments, 48 hour meetings, initial, quarterly and annual assessments.

Long term care experience is preferred, however, we are willing to train the right candidate!

Requirements:

· LTC/Nursing Home Experience preferred

· Licensed Required

· Per MA Department of Public Health regulations, candidates must be fully vaccinated against COVID-19.

Job Type: Full-time

Pay: $37.00 - $42.00 per hour
