Service Coordinator

Responsibilities

Outline below are basic broad categories describing the Service Coordinator job responsibilities. The responsibilities include but are not limited to the following:

Daily-

1. Check on frail residents by phone or home visits

2. Call any residents that are hospitalized

3. Schedule needed transportation for residents

4. Keep records of any action in each residents file

5. Contact health agencies if needed

6. Keep property manager informed

Weekly-

1. Assist residents as needed for any programs

2. Email property manager activity report

Monthly-

1. Conduct resident needs assessment

2. Locate and schedule speakers/presenters for the residents

3. Check for training opportunities. Keep training log.

4. Make sure all new residents have an assessment completed within the first month of tenancy

5. Gather suggestions and ideas for resident participation and involvement for new and desired programs.

Vacation & Personal

Time professionally managed by Community Management Corporation. EOE

Part Time 15 hours per week

Job Type: Part-time

Pay: $15.00 per hour
