The Transition Coordinator will help provide a full range of academic, pre-vocational, vocational, independent living and community based instruction services to MSDB staff, including assessment, individual and group instruction, and work with/refer to outside agencies as appropriate. He/she will influence the development of a safe, successful learning environment, positive post-secondary experiences, and positive school culture by partnering with teachers, administrators, school staff, external agencies, and members of the communities in which our campus students come from. He/she will demonstrate knowledge of and support of, Montana School for the Deaf and the Blind mission, vision, belief statements, standards, policies and procedures, operating instructions, confidentiality standards, and the code of ethical behavior.

Essential Duties or Responsibilities:

A. Community Outreach and Training 40%

  • Establish and maintain relationships with local businesses in the community for student access to vocational and daily living opportunities
  • Work with school staff and administrators to establish and build site based vocational experiences
  • Work collaboratively with school counselor and postsecondary education institutions to encourage students with disabilities to consider postsecondary education, make selections based on interests and preferences, and facilitate application activities to the Department of Rehabilitation and other agencies that provide support to students with disabilities in postsecondary education
  • Works collaboratively with school representatives to provide information sessions for students and parents, conduct eligibility and application activities, and work to transition students from school services and supports to adult services and supports
  • Ability to work collaboratively with school representatives, community partners, parents, and students as part of the coordinated set of activities and transition program components

B. Work Program 20%

  • Work with MSDB school and cottage staff to coordinate supervision and evaluation of students in vocational training sites
  • Help secure in-school and community job placements for students ages 14-21
  • Provide guidance to students in the areas to support vocational placement and motivate students by utilizing individualized instructional methods that meet individual student needs.
  • Provide guidance on how to apply the student’s knowledge to real-life situations as they transition out of school-aged programming.

C. Individualized Transition Plan (ITP) and Individualized Education Plan (IEP) 20%

  • Complete the Transition Services Component of the IEP and contribute to the Summary of Performance
  • Demonstrate the ability to assist with IEP Programming, specifically for transition aged students at MSDB including:
    • Assist in the development of ITP and IEP goals, and course of study in the areas of postsecondary education, employment, and independent living
    • Regularly progress monitor IEP goals through use of an online Brolly system
    • Regular attendance at IEP meetings for the purpose of reporting student progress
    • Work collaboratively with the IEP team to help conduct comprehensive transition assessments, and write, implement and progress monitor Individual Transition Plans in accordance with the IDEA
  • Facilitate the achievement of the desired post-school outcomes for students with disabilities
  • Demonstrate knowledge of transition assessment and planning, adult agencies, work based learning, and culturally responsive transition practices

D. Parent / Family Coordination 15%

  • Work with school staff and administrators to provide parent training and parent involvement activities that support transition for their child
  • Conduct individualized transition planning meetings with parents and/or students
  • Educate parents by distributing information on transition issues, hosting parent information meetings, and scheduling tours of the LEAP facilities
  • Respond promptly to questions and parent concerns

E. Other Duties as Assigned 5%


Experience Required

  • 1 year(s): Experience working with transition services for deaf and hard of hearing, visually impaired, and deaf-blind students
  • 1 year(s): Experience working with deaf and hard of hearing, visually impaired, and deaf-blind students in an educational setting

Education Required

  • Bachelors or better in Social Work or related field, OR
  • Bachelors or better in Education or related field
