Requirements:
Must have experience in Medicare remote patient monitoring services for CCM patients
Highly proficient in EMR and Microsoft Suite.
Fast paced team. Must be very comfortable using new technology
Current and Valid MA license

Come join our quickly growing Remote Therapeutic Monitoring team. Monitor rheumatology patients at 1 dedicated clinical site.
What You Will Do:
- Provide observation and surveillance of assigned patients.
- Monitor patients by use of clinical dashboards and phone calls.
- Communicate and/or instruct patients by use of 2-way audio device.
- Notify clinical staff of any changes in patient condition and/or safety risk level.
- Complete documentation of observations of patient activities, patient behaviors and other safety & comfort related issues.
