Non-acute care is offered through various affiliates, including the Thompson House for rehabilitation and skilled nursing services, and the Home Care organizations.Summary:
Provides case management services to a chronic mentally challenged population; delivers the bulk of individual face-to-face treatment in the community setting (for ICOTT: 60 % face to face, 75% in community, and for Case Management / Community Support Program: 60 % face to face, 60% in community) · Adjusts level of contact and interventions to stabilize the client in the community and avoid or minimize hospitalization · Provides a range of skill-building interventions to facilitate the client progressing toward recovery / re-integration into the community. · Provides education to Client and families / significant others related to managing symptoms, coping skills, anger management, socially appropriate behavior, medication compliance, accessing the system, and available resources. · Provides transportation of patients to medical / psychiatric appointments and setting up / teaching alternative means of transportation, assisting with entitlements, housing, obtaining food / furnishings, supporting clients through legal proceedings, and helping clients access community resources. · Facilitates the client's movement along the continuum of care from more intensive to less intensive levels of care by implementing the treatment and discharge plans, by crisis intervention, and through resource utilization and management · Monitors compliance taking medications in medication boxes.
Responsibilities:
1. Community Outreach Criteria A: Assists clients in finding housing by involving them in the process. Community Outreach Criteria B: Facilitates access to community resources, agencies, and services. Community Outreach Criteria C: Participates in treatment planning and makes appropriate referrals as part of the continuum of care and/or discharge planning. Community Outreach Criteria D: Assists clients with entitlements, finances and with applications for services in the community.
2. Facilitates Psychosocial Rehabilitation Criteria A: Collaborates with staff in other settings, such as shelters, TRP, halfway houses, other hospital units, and / or the forensic services (probation, parole, and the court system) regarding client's status and treatment goals / objectives. Facilitates Psychosocial Rehabilitation Criteria B: Provides clients with education and training on independent living skills, including ADLs. Facilitates Psychosocial Rehabilitation Criteria C: Transports clients safely to and from appointments (medical, social services, etc.), assists clients to secure food, clothing, furnishings, attempts travel training to facilitate independence, and encourages reintegration into the community by addressing socialization skills. Facilitates Psychosocial Rehabilitation Criteria D: Promotes independence by fostering an environment conducive to learning.
3. Admissions Intake and Treatment Planning Criteria A: Conducts a "biopsychosocial" intake, collects information for "Co-Sig" screening for co-occurring disorders, and collects list of medications for medication reconciliation process. Admissions Intake and Treatment Planning Criteria B: Demonstrates knowledge and ability to recognize mental status and modified global assessment of functioning. Admissions Intake and Treatment Planning Criteria C: Participates actively in the admission, treatment planning, and discharge / transfer phase of the client's care by collaborating with other team members and care givers. Involves the client in establishing treatment goals and objectives and in any revisions in treatment plans. Admissions Intake and Treatment Planning Criteria D: Maintains daily log of direct / indirect patient care activities and submits on a weekly basis for DMHAS (DPAS) statistical data collection.
4. Therapeutic Interventions Criteria A: Screens on admission and as needed and monitors the client for suicidal thoughts or actions, explores therapeutic options to preseve client's safety. Therapeutic Interventions Criteria B: Conducts a risk assessment, recognizing potential for violence or self-harm, including intent, plan, previous attempts, family history, and takes appropriate actions. Therapeutic Interventions Criteria C: Recognizes signs / symptoms of abuse, neglect and family violence. Therapeutic Interventions Criteria D: Interacts with the client to defuse angry and / or aggressive verbalizations, outbursts, or actions.
5. Provides supportive services Criteria A: Establishes daily or weekly contact as needed to provide more intensive therapeutic support during times of crisis, or mental decompensation. Provides supportive services Criteria B: Provides concrete supportive therapeutic interventions to severely mentally challenged patients. Provides supportive services Criteria C: Responds to crisis and communicates with other care givers, family members or significant others if allowed, regarding the client's changes in mental status and / or behaviors. Provides supportive services Criteria D: Collaborates with treatment team on treatment goals, medication issues, and changes which impact patient's quality of life.
6. Documentation Criteria A: Demonstrates ability to document observations and therapeutic interventions objectively. Documentation Criteria B: Documents treatment plans, biopsychosocial assessments, changes in mental status or behaviors, community outreach, crisis interventions, and the client's progress per documentation standards. Documentation Criteria C: Maintains the integrity of the medical records by complying with documentation standards and required screenings. Documentation Criteria D: Completes documentation in a timely fashion.
7. Professional Responsibility Criteria A: Participates in Staff and clinical team meetings. Professional Responsibility Criteria B: Seeks opportunities to expand knowledge base by reading articles, and by attending staff development / continuing education programs. Professional Responsibility Criteria C: Maintains and updates Educational Record and complies with annual hospital - wide and mandated training programs. Professional Responsibility Criteria D: Maintains client confidentiality (defined exceptions for suicidality and imminent danger to others) and confidentiality of records.
Other information:
Required: BA or equivalent education and experience working with the chronic mentally challenged population. Must be able to deal with a high level of acuity in a psychiatric outpatient setting. Frequent patient contact / face-to-face interaction in the hospital and in the community. Must be able to make independent decisions, balance multiple priorities, manage stressful situations, assess risk, convey a “can do” attitude and a message of hope that recovery is possible, and facilitate the client taking steps toward greater functioning. Bilingual / Multicultural skills preferred.
Minimum Experience: zero to six months
Desired: Must have excellent communication skills, verbal and written; must be able to comply with documentation standards from Norwalk Hospital and DMHAS (Department of Mental Health and Addiction Services). Must be perceived as accepting and nonjudgmental, convey respect and empathy; and act as client advocate . High energy level required, with ability to work under pressure and with the chronic, indigent, and forensic population. Must have the ability to exercise sound judgment in crisis situations. Connecticut Driver's license in good standing required; must submit annual proof of Driver’s license and notify supervisor immediately if any action taken against driver’s license (revocation or suspension). Ability and flexibility in schedule to work evenings and weekends on a rotating basis. Other Duties as required.
Computer Skills Required: PC proficiency in Lotus, Window and Excel, Windows.
