Summary:- Requires a 1 year commitment to Confluence Health and be an external applicant.
Acts as an aide for Hospice and Home Health seeing patients in their home environment under the supervision of a registered nurse or licensed therapist in accordance with the physician’s plan of care to include assisting patients with activities of daily living, providing basic nursing care to all patients, and maintaining a safe and clean environment.
Position Reports To: Hospice Supervisor and Agency Leadership
- Drives to patient’s place of residence.
- Perform related assignments and tasks as requested to promote positive visitor, family, and staff relations.
- Reports any observed change in patient condition or behavior promptly to the nurse. Ensures patient care tasks are completed timely.
- Performs or assists with duties as outlined in the Home Health and Hospice state and federal regulations and in the CNA Scope of Practice under the direction of the Registered Nurse.
- Demonstrates competence in providing care routines in accordance with established nursing care procedure and plan of care to ensure patients' comfort and safety.
- Takes and records vital signs, height, weight, intake and output, and other measurements as applicable, in an accurate and timely manner. Reports all variations from normal to the RN in charge of the patient.
- Ambulates/Transfers patients using proper body mechanics and patient safety techniques.
- Assists nurse in treatment procedures as needed.
- Documents accurately and timely in the patient medical record.
- Maintains a clean and safe environment for patient and staff according to hospital and unit policies.
- Uses two patient identifiers before patient care.
- Actively participates in quality improvement programs.
- Adheres to Confluence Health and Central Washington Hospital/Wenatchee Valley Hospital policies and Home Health and Hospice standards as applicable.
- Required to maintain active Basic Life Support (CPR) certification.
- Other duties as assigned.
- Practices at a fully competent level and has one year of experience as a CNA.
- Graduate of an accredited nursing assistant program and:
- High School Diploma or GED.
- Active WA certified CNA license.
- Since most recent completion of the program there must not have been a 24-month lapse in providing aide service prior to hire date.
- Must possess basic computer skills related to Windows navigation, mouse usage, keyboarding, email communication and password management.
- Annually completes competency/certification requirements.
- Maintains a valid WA driver’s license and vehicle insurance.

- Has three years of experience as a CNA.
