Do you have experience working direct care in the mental health field / working with vulnerable adults? The CAREGIVERS (DSPs) of our area persons with disabilities home LOVE their residents. In speaking to these caregivers, they share that they learn so much from their residents every day. Some additional duties in this LEAD role are to set appointments for residents, ensure all resident medications are in house with current prescriptions, coordinate residents' doctor appointments, following up with care team (nurses and program director) as well as work the floor.

Coordinates health programs and/or Person Centered Plans and provide direct care to individuals living within assigned Center.
Duties and Responsibilities
- Assist in the coordination of medical and health programs and services for residents including but not limited to scheduling medical appointments, ordering medications, and obtaining physician orders; assure accurate and timely documentation of same
- Provide hands-on care and assistance to persons served, including but not limited to mobility/lifting/ transferring; bathing, personal hygiene and grooming, laundry, meal preparation and mealtime assistance. Perform First Aid as necessary
- Assist persons served with activities in daily living including but not limited to household tasks, communication skills, and activities in the community
- Assist in development of individuals’ social and recreational interests and skills through appropriate modeling and supportive activities
- Pass medications in accordance with Person Centered Plan and Adult Foster Care Licensing rules for the State of Michigan
- Complete and document consumer’s home-based programs as written by Community Mental Health (CMH) personnel
- Provide care and complete duties in compliance with Adult Foster Care Licensing rules for the state
- Cooperate with other Center staff to assist in meeting the needs of individuals in the Center
- Participate with Manager, Case Manager and other assigned professional support staff in the development and implementation of individual program, supports and services
- Transport resident in Samaritas owned vehicle: assure basic cleanliness and care of vehicle
Job Qualifications
Education, Training, and Licensure/Certification
- High school diploma or equivalent preferred
- Satisfactory completion of DCH Residential Program Training Requirements
- Attain and maintain all required certifications (i.e., CPR, First Aid, Blood borne Pathogens, etc.)
- Complete required CMH Group home training
- Valid unrestricted driver’s license with good driving record. Proof of valid automobile insurance if required by program/center
Experience
- Experience in a residential setting for people with developmental disabilities or mental health preferred
