JOB SUMMARY:
The Service Facilitator is responsible for providing case management services to children with severe emotional disabilities (SED) and their families who are at risk of psychiatric inpatient hospitalization or other out-of-home placements. Case management services will incorporate the wraparound model of service delivery in which supports in the home, school, community, and other areas of the child’s life will be encouraged to take a team approach to assist the child and the family in meeting their needs. Case management approach will include a strength-based and person-centered approach to service delivery.
Salary: $21.63 per hour
Plus $1,000 sign-on bonus!
$1,000 raise every 6 months, for the first 2 years of employment!
Job Location: Office is based at the Kenosha County Job Center with flexibility to work primarily remote. Hybrid model of time spent in-home with families, in the office/remote, and in the community. Must be able to meet in-person with clients in Kenosha County, Wisconsin.

Key Responsibilities:
- Develop, implement, evaluate, and revise outcome oriented and strength based treatment plan as changes occur.
- Develop a family support team comprised of formal and informal supports.
- Facilitate team meetings with service providers, family members, and family supports in which progress of goals and services are monitored.
- Develop a crisis/safety plan for the client and family.
- Exercise judgment to intervene as an advocate for clients in emergency and crisis situations.
- Attend client/family court proceedings, IEP meetings, psychiatry appointments, or other appointments that may require family support or advocacy.
- Utilize advanced knowledge of services and resources in the community and be willing to research new and unknown resources to assist the client and family in meeting their goals.
- Safely transport clients as needed.
- Must have reliable transportation on a daily basis, a valid driver’s license and automobile insurance.
- Bachelor’s Degree in Social Work, Psychology, Criminal Justice, or other related fields.
- One year experience working with the severely emotionally disabled population is preferred, but not required.
