Job Summary:

The Primary Purpose of the Infection Preventionist nurse is to support the Director of Nursing (DON) and Clinical team in providing adequate and appropriate care for the residents we serve. We strive to meet or exceed the expectations of residents, families, regulators, and team members. This position is also responsible for the coordination and oversight of our Infection Control program in coordination with the Quality Department as needed.

The Infection Preventionist makes frequent rounds every shift to help monitor and evaluate the infection control practices and quality measure initiatives, creating a culture of continued improvement. The IP nurse ensures care delivery is consistent with the individualized plan of care for each resident, and the current policies or practices in place. As delegated by the DON or ADON: monitors, tracks and audits infections, immunizations and employee illness through reports and mapping to evaluate for trends. If trends are noted, creates a plan for education and/or process improvement. The IP nurse will work with physicians/all providers, and the Interdisciplinary team to ensure a well-coordinated infection control, antibiotic stewardship program and QAPI (Quality Assurance and Performance Improvement) program. The IP nurse will support the clinical leader on call rotation.

• 
  Graduate of an accredited associate degree or higher Nursing program
• Current active and unrestricted RN License in State of Michigan
• Infection Preventionist Certification preferred or expected to acquire within 3 months of hire through the CDC
• Experience with QAPI and Quality Measures preferred
• Knowledge and experience in Skilled Nursing Facility clinical operations or other appropriate health care setting- preferred
• Experience with LARA / CMS annual certification survey process preferred
• Working knowledge of electronic medical records, PCC preferred
• Ability to assess care and ensure adequate and appropriate care is delivered
• Ability to plan, organize, persuade, and solve problems - creating a positive culture that inspires full engagement of team members
