The Niagara Hospice Medical Social Worker provides clinical social work services within the scope of practice to patients/families admitted to the Hospice program.

Specific Duties/Responsibilities:

Provides but is not limited to the following patient/family services:

1. Assist patient in facilitating their goals; per patient choice

2. Assist patients and families in:

  • addressing their anticipatory grief issues as needed
  • accessing insurance information as need
  • accessing nursing home information as needed
  • providing volunteer services information
  • health teaching/disease progression

3. Conduct suicide prevention/lethality assessment

4. Mediate and advocate for patient choices

5. Assess families for potential complicated mourning, disenfranchised grief, or psychopathological history, and refer for pre-bereavement and /or mental health services as warranted

6. Identify and coordinate a plan to meet the psychosocial needs and services with other patient/family needs and services being addressed by the team

7. Participate in assessments, admissions, discharge planning, service availability, treatment plan design, implementation of services, review of services, and evaluation responsibilities of the team.

8. Provide special awareness and assistance in the identification and dealing of psychosocial stresses of patients and families in accordance with their own cultural and spiritual beliefs as they go through anticipatory grief, the dying process, and death

9. Establish necessary service provider linkages with allied health and social service agencies on behalf of the Hospice patient/family, with appropriate follow-up and advocacy

10. Maintain confidential, accurate, timely, comprehensive, legible, detailed, and professionally written records as required by Niagara Hospice and consistent with State, Federal, Third party payor regulations

11. Maintains a high level of professional competence, with ongoing development of clinical and interdisciplinary issues related to death, dying and bereavement

12. Coordinate the delivery of social work services to maximize effectiveness and eliminate duplication with contracted and/or cooperating social service units of hospitals, county DSS, and other public and private agencies

13. Keep the Director of Hospice Homecare Services and Medical Social Worker Mentor apprised of any difficulties encountered during visits

14. Serve as the Niagara Hospice Social Work Representative in the Community.

15. Perform other functions, attend meetings, conferences, and in-services and share in the support, development and community education of the Hospice concept and philosophy, as requested

16. Participate in Social Work on-call system.

17. Report suspected abuse/neglect of adult/children to the local county department of social services or the appropriate authorities as per state law

18. Serve as a Niagara Hospice IDG member, and promote and adhere to the elements of the corporate compliance program.

Qualifications:

Education Requirements:

1. A minimum of a Masters degree in Social Work from a college or university accredited by the Council on Social Work Education.

2. A minimum of one year experience in a social service agency adhering to acceptable standards, preferred.

Licensure/Certification:

Must have a valid Driver’s License
