FLSA Exemption Status: Non-Exempt
Reports to: Director, Nursing Services
Schedule: Monday - Friday 8 am - 4:30 pm (No weekends or holidays)

InnovAge Summary:
InnovAge’s Program of All-inclusive Care for the Elderly (PACE) is an alternative to nursing facilities. Seniors receive customized healthcare and social support at a nearby PACE center. Each PACE participant has a team of medical experts dedicated to providing personalized healthcare and support to help them age at home.
Position Summary:
The Certified Nurse Aide works at our Day Center and is responsible for providing routine care and assisting participants or clients with activities of daily living, and restorative and supportive care. Physical attendance at an InnovAge facility and/or an InnovAge participant home is essential to performing the job duties.
Essential Job Duties:
- Follows plan of care and assists participants with activities of daily living such as bathing, dressing, grooming, hair care, mouth care, eating, hand, and toileting as assigned or needed. Documents care provided in accordance with established policies, guidelines, and applicable regulations
- Carries out, reports and documents care given in an effective, timely manner as observed by the RN and through periodic record reviews.
- Participates in the care planning of participants through cooperation with multidisciplinary teams.
- Monitors participants’ health status by taking vital signs and weights (monthly).
- Monitors appetite in addition to looking for changes in functional ability and checking skin condition while assisting with toileting.
- Monitors/assists participants in a courteous manner with meals and provides assistance with feeding as necessary.
- Assists participants with restorative and supportive care such as ambulating, positioning, transferring, skin care, catheters, filling oxygen, and providing other aides as assigned or needed.
- Monitors participants’ health status such as vital signs, weight, skin condition, appetite, and changes in functional ability, and reports pertinent information to the clinical supervisor in a timely manner.
- Reports any variance, accident, or unusual occurrence to the appropriate personnel.
- Attends in-service and continuing education programs as scheduled and necessary.
- Assist drivers with the loading and unloading of participants from vehicles.
- Assist the transportation department with participants’ take-home supplies.
- May rotate into In-Home Services as a Homecare C.N.A. as applicable and deemed necessary by the ADHC Services Manager.
- Creates and participates in group(s) and one-on-one activities in accordance with the daily activity calendar when needed.
- Participates within the interdisciplinary team in the formulation of Plans of Care for InnovAge PACE program participants, as well as in other interdisciplinary team settings that plan, coordinate, and monitor the care of InnovAge PACE program participants.
- Additional work functions and duties may be assigned.
Required Qualifications:
- High School Diploma or GED
- A current and valid state-issued Certified Nursing Assistant (CNA) license is required
- Current CPR certification or the ability to obtain CPR certification is required.
- Require personal transportation, current state-issued driver’s license, good driving record, and auto insurance as required by law.
- A minimum of one year of experience working with the frail or elderly is required.
Preferred Qualifications:
- Experience in a care setting for frail and elderly patients participating in a group environment preferred
