The primary purpose of your job position assists residents in the community with all activities of daily care and services in accordance with the resident’s plan
- Provide assistance and general resident care, including, but not limited to :social interaction, meal assistance , bathing, toileting and housekeeping
- Follow care plan, including monitoring resident inputs, output, vitals, behavior, safety concerns and any other changes
- As needed , be an active participant in life enrichment activities
- Documentation of all input, output, vitals, behavior and or safety as needed in client file
- Contribute to employee communication log to ensure smooth transition of necessary information from one shift to the other
- Able to do occasional lifting of up to 50 lbs.
- Able to work flexible schedule , including evenings and/or weekends
- Other duties maybe required

Minimum Qualifications
- Successful completion of a Certified Nursing Assistant program in the State of employment
- Current CPR/ First Aid certification (if applicable)
Preferred Qualifications
- Previous experience working in a memory care or assisted living community
Physical Working Requirements
- Ability to travel using personal vehicle, including at night
- Able to do occasional lifting of up to 50 lbs.
- Able to work flexible schedule, including evenings and/or weekends
- Team Member is subject to outside environment conditions occasionally, inside conditions frequently, with protection from weather conditions but not necessarily from temperature change
- Tasks assigned to this position may involve potential and/or direct exposure to blood, body fluids, infectious diseases, air contaminants, and hazardous chemicals
- Must be able to read, write, speak and understand the English language
