DEPARTMENT DESCRIPTION: CHA PACE is a Program of All Inclusive Care for the Elderly (PACE) at Cambridge Health Alliance (CHA). CHA PACE serves adults 55 years and older with chronic medical and/or mental health difficulties with the goals of achieving the highest level of community independence and preventing nursing home placement. CHA PACE’s high functioning interdisciplinary team works to fully integrate and coordinate medical and behavioral health care, community resources, and in home support services.
Job Purpose:
The Occupational Therapy role focuses on participating on the interdisciplinary team level to support and educate therapeutic needs of a participant, provide input to participant care plans, and maintain a caseload of participants that can be treated both in the home and in an outpatient gym setting located in our Adult Day Health Center. We are a unique preventative care model compared to the reactive health care systems that most home care agencies adhere to.
In Accordance with CHA policies and procedures, federal and state regulations, professional standards of care, and AOTA guidelines and code of ethics, the Occupational Therapist evaluates and treats program participants in their home as well as the PACE Center clinic with a focus on quality care, service excellence, cultural competence, and performance improvement. This position is team based home care without the utilization of OASIS typically used in home care visits.
Qualifications/Requirements
Education/Training: Bachelors, Masters, or Doctoral Degree in Occupational Therapy
Licensure: Massachusetts OT License
Certifications: AHABLS for Healthcare Providers
Work Experience: 0 years, 1+ preferred
Physical Skills:
Ability to perform all evaluation and treatment procedures related to job.
Sufficient strength, mobility, visual and auditory acuity to safely and independently guard and potentially support patients while ambulating on flat surfaces and stairs.
Sufficient strength to carry, push, or lift patient care equipment as needed.
Physical ability to respond to emergency patient care needs, i.e.: administer CPR, evacuation procedures.
Physical dexterity and psychomotor skills to operate common rehabilitation equipment.
Visual, auditory, and communication capabilities sufficient to read information and respond to written and auditory input.
Ability to document patient care information on keyboard, by hand, and/or with other devices.
If home care responsibilities:
Must possess valid driver’s license, and have reliable transportation
Be able to follow all local and state laws for the operation of a motor vehicle and public safety. Be able to safely navigate through seasonal environmental conditions
Mental Skills:
Cognitive ability to observe, perform, interpret, and analyze evaluations and treatments as they relate to patient care
Ability to read and communicate
Capacity to respond in a rational, appropriate manner to emergency situations
Ability to manage time effectively and prioritize responsibilities as needed
Good organizational skills
Ability to work without direct supervision
Able to work in demanding, stressful environment with sufficient concentration to effectively accomplish patient treatment goals even with interruptions and distractions
Working Conditions and Physical Environment:
Inpatient and outpatient units: acute care hospital, psychiatric units, Private homes, facilities
Community clinics
Tile/carpeted floors, fluorescent, incandescent, and natural lighting, controlled air temp.
Various workstation heights and or occasionally crowded department
Potentially noisy or chaotic environment
Diverse patient population of various age/gender/race/religious beliefs
If home care/CHA PACE responsibilities:
Patient’s private residence: house, apartment, condominium, etc.
Various flooring surfaces, stairs, narrow hallways
