Program: Grandparent Support Program

Position: Grandparent Support Specialist

Salary: $21/hour

Catholic Charities is seeking a Grandparent Support Specialist in the Grandparent Support Program. This is a full-time position. The Grandparent Support Program aims to enhance early care services provided by grandparents in underserved populations, emphasizing serving in communities outside of Stockton but within San Joaquin County. The program will provide guidance, coaching, and access to health and social services to enhance grandparents’ caregiving skills and help ensure the children in their care have a safe and nurturing environment that allows them to thrive and grow.

Responsibilities:

· Conducting targeted outreach and utilizing culturally and linguistically appropriate program strategies to promote the representation of targeted groups.

· Provide quality care coordination for a community for grandparent providers caring for grandchild/ren ages 0 to 5. In addition, provide case management for a cohort of grandparents caring for a grandchild/ren ages 0 to 5yrs old.

· Establish a quality growth plan focused on increasing the quality of care they provide to the child/ren in their care.

· Facilitate Grandparent Education Workshops throughout the duration of the program.

· Conduct at least two monthly home visits per Family, Friend, and Neighbor (FFN).

Qualifications:

· AA Degree and three years’ experience in social services or related behavioral health environment.

· Experience with Microsoft Word, Excel, and PowerPoint.

· Bilingual (Spanish).

· Available to work evenings and weekends.
