The Homelessness Prevention Program (HPP), a program of Catholic Charities, provides homelessness prevention assistance to families and singles in San Francisco to help them maintain or obtain permanent housing. HPP assists families in permanent housing who are in danger of eviction or who need help to move to a more sustainable rental situation. Service components provided include needs assessment, budget planning, landlord mediation, referrals to tenancy counseling and legal assistance, short-term case management, necessary referrals, and access to financial/rental assistance.

The Intake and Information Referral Coordinator is the initial point of contact for HPP and other homelessness-related programs at Catholic Charities. As such this position acts as a welcoming and knowledgeable portal to accessing services. The Intake Coordinator listens and assesses each individual situation to determine the most appropriate services to stabilize the family or individual's housing situation. He/she makes appointments for HPP staff and coordinates referrals to other agencies. This position will work extensively with the community agencies which are collaborative partners with our program.

ESSENTIAL DUTIES & RESPONSIBILITIES

  • Under the direction of the Program Manager provides coordination of services, including assessment, crisis intervention and intake services for families and individuals, explains program regulations and guidelines and ensures that clients are provided with appropriate services from the agency and community providers.
  • Sets up all appointments for HPP staff and enters all intake information into CODI.
  • Maintain proper client files and program documentation; compile reports as requested (monthly, quarterly).
  • Provide accurate information and referrals to clients: maintain resource knowledge and refer clients to the appropriate programs of Catholic Charities and/or other agencies; enter I & R data into CODI.
  • Provide support to the Program Manager/Director, and other Catholic Charities programs and newly hired program staff as needed.
  • Follow-up where necessary to ensure quality service delivery.
  • Participate in all assigned staff and organizational meetings and office functions.
  • Assist Program Manager/Director in the development, maintenance, and improvement of standards for quality client service according to client and partner feedback.
  • Act as liaison between Catholic Charities HPP program and the community agencies which are collaborative partners; follow up on agency referrals and respond in a timely manner.
  • Organize orientations for collaborative partner staff and potential qualifying families as necessary.
  • Provide ongoing training to partners as needed to ensure smooth working relationships and accurate documentation.
  • Compile reports as requested.
  • Enter and lose cases in the HSA database.
  • Intake and assessment for all drop-in clients to 990 Eddy St San Francisco Ca 94109. Location; Establish eligibility and refer clients internally or externally. Provide back up to support to the front desk.

QUALIFICATIONS

Education & Experience:

  • AA or BA degree, plus two years of experience preferred.
  • Experience with San Francisco housing resources and public benefits.
  • Other relevant experience will be considered.
  • Experience working with persons in crisis.

Knowledge, Skills & Abilities:

  • Spanish Language proficiency requires.
  • Thorough knowledge of San Francisco Service Providers (e.g. substance abuse, housing, financial assistance, government entitlement programs, and money management services) required.
  • Excellent interpersonal and communication skills.
  • Able to listen actively and communicate effectively with individuals and groups and be sensitive to the client population, organize own work, and follow-through independently.
  • Detail and achievement Oriented.
