Overview:
To assist all members of the Health Services Department in ensuring optimal health care delivery to participants of Seven Hills Rhode Island. Working under the direction of nursing, the Health Services Assistant collaborates with health care providers, agency team members, families and individuals supported to ensure optimal health care delivery. Responsibilities:
Rate: $22.44/hour
$1,000 sign-on bonus
To assist all members of the Health Services Department in ensuring optimal health care delivery to participants of Seven Hills Rhode Island. Working under the direction of nursing, the Health Services Assistant collaborates with health care providers, agency team members, families and individuals supported to ensure optimal health care delivery.
Schedules and facilitates health care appointments as directed by primary nurse.
Schedules with program managers and coordinators, secures, and accompaniment for appointments.
Schedules all psychiatry/behavioral health clinic appointments.
Maintains documentation of medical appointments, testing, prescriptions, annual physicals and other health related information.
Assists nurse to maintain health care records.
Acts as liaison between health care providers, agency staff and person supported.
Orders and maintains emergency, first aid and health care supplies and completes monthly safety checks.
Responsible for the overall physical, psychological and emotional health and safety of the participant.
Models healthy behavior and positive social skills.
Provides supports as written in the individual’s support plan that may include personal care, physical lifting and therapeutic holds.
Provides feedback to team members and/or supervisors as to effectiveness of individual plans.
Assists with an individual’s environment such as cleaning, laundry, shopping, cooking, errands and budgeting.
Facilitates and maintains family contacts and community involvement.
Encourages the development of new relationships and contacts by assisting people to be active in their communities and workplace.
Monitors the dietary needs of the program participant.
Communicates effectively in verbal and written format. Communication is to be given and received and is every employee’s responsibility.
Actively participates in improving communication with participants by learning, encouraging and actively utilizing an individual’s modes of communication.
When applicable, follows doctors’ orders and ensures that medications and treatments are administered following proper medical procedures.
Recognizes, reports and assists with medical crises as they occur.
May assist in the vocational evaluation process.
May support vocational programming including payroll for hours worked, merit ratings, referrals and job coaching.
Performs work in compliance with Human Rights regulations.
May work independently of direct supervision or, at times, alone.
Maintains confidentiality and HIPAA compliance.
Maintains and takes responsibility for certifications and all trainings as required, respectful of management leadership at all times.
Contributes to the positive morale by representing the agency through professional communication and interactions.
Implements and fosters the principles of self advocacy which promote an independent lifestyle and respect for program participants.
Assists people with learning about and advocating for their personal rights and understanding the responsibilities that accompany those rights.
Arrives punctually and participates in all required meetings and trainings.
Follows all established policies and procedures and documents accordingly.
Drives people to destinations necessary for them to experience a full and diversified life, following all safety and legal regulations.
May also participate in a program participant’s chosen activity.

Two (2) years experience in the field of developmental disabilities. One (1) year in the field of health care. High School Diploma or GED required. Knowledge of accepted practice guidelines for preventative health care and medical terminology. Effective oral and written communication skills. Effective computer skills including MS Word, and basic Excel. Able to work with minimal supervision with flexibility. Valid driver’s license and insured. Acceptable background investigation results. CPR/First Aid Instructor preferred.
