Our CAT and OnTrack programs are innovative multi-disciplinary teams serving adolescents, young adults and their families. Each program provides community-based, coordinated, and comprehensive services.

The CAT team provides family centered and culturally competent community support and treatment to youth and young adults to help them live successfully in the community. Services are individualized.

The OnTrack team works together in supporting adolescents and young adults who have experienced an initial psychotic episode in the early stages of mental health issues. The OnTrack Team also supports the families of those they serve and focuses on a modern and therapeutic approach.

POSITION DETAILS:

- Staff Nursing responsibilities for CAT and OnTrack Clients.

- Working with APRN/Psychiatrist to manage psychiatric evaluation and medication management appointments.

- Ensuring follow-up med related activities are completed and medication appointments are scheduled.

- Assessing for psychiatric symptoms, responses to medications and medication side effects.

- Educating Clients and families on medication adherence and side effect management.

- Providing lab work, assisting with processing and obtaining/delivering results.

- Overseeing medication monitoring (e.g., Depakote, Lithium, Clozapine) including labs and prescriptions, etc.

- Responsible for administration of injectable medications for Clients.

- Completing prior medication authorizations as needed.

- Managing onsite medication supply per agency standards.

- Coordinating Clients' care with primary care provider and other healthcare providers (referrals, medical records, etc.) to ensure continuity of services.

- Providing wellness education for healthy eating, exercise/fitness, stress management, pregnancy prevention, safe sex, etc.

- Potentially providing in-home or community support to Clients and families.

- Managing mental health crisis situations for Clients.

- Basic First Aid and managing onsite medical emergencies.

- Horseshoe Campus liaison for Infection Control.

- Other duties as required by the Supervisor.

TO JOIN OUR ELITE CAT & ONTRACK TEAMS:

- Clear and active FL Nursing License (LPN or RN) = Required

- Graduation from accredited Nursing Program = Required

- Spanish or Creole Bilingual = Preferred

- Nursing experience related to serious/persistent/escalating mental health issues = Required

- Experience working with children/youth/young adults in a community-based mental health setting = Required

- Current State of Florida Driver's License with clean record = Required

- Demonstrated abilities in the proper management of time, communication and information = Required

- Comfortability working both independently and within a multidisciplinary team = Required

- Natural ability to work well with a diverse group of Clients, coworkers and community partners = Required

- Microsoft Office 365 (Including Teams/Video Conferencing) Electronic Medical Record = Strongly Preferred

- Maintaining nursing certification and compliance with all trainings and immunizations = Required

- Ability to pass a Level II DCF Mental Health Fingerprinting, Drug Screen and Local Criminal Check = Required

LOGISTICS & COMPENSATION:

- Full-Time Hourly/Non-Exempt position
~ Monday-Thursday, Four X 10-hour days
