Requirements:

• Must have an active Florida RN License
• Bachelor of Science in Nursing degree is preferred
• Minimum of 3 years of nursing experience required
• Case Management experience is preferred, but not required

Your Main Duties and Responsibilities:

This full-time position requires you to manage a team of patients, typically a caseload of between 15-18 patients. You must be flexible to visit your patient's PRN as special needs arise. As the RN Case Manager, you will be responsible for ensuring all patients' needs are met and providing them and their families with resources. You will utilize assessments, interventions, and education to maximize patient comfort and enhance the quality of the patient and family.

• 216 hours of PTO annually and additional paid sick leave, usable after your first 90 days
