We are searching for a Home Care Aide to perform patient care activities in accordance with Southcoast VNA policies and procedures, communicate pertinent patient information and unusual findings to appropriate Clinical personnel, complete all required documentation accurately and in a timely manner and provide exceptional and caring customer service to patient and family.

• Hours: 40hrs
• Shift: Day shift with weekend and holiday rotation; 8:00 AM-4:30 PM
• Travel: This is a home care position. Travel is required.

Locations:

Team 1: Dighton, Tiverton, Seekonk, Swansea

Team 2: Fall River area

Team 3: Wareham, New Bedford, Rochester, Buzzards Bay

Team 4- Dartmouth, New Bedford, Fairhaven, Mattpoisett

Team 5- Westport, Raynham, Taunton, Berkley

Education and/or Experience Requirements:

• Equal to completion of four years of high school plus specialized courses and training.
• CNA certification required. MA/RI licensed (RI license obtained within 90 days of hire)
• CPR certification required. Medical Assistant, EMT or Paramedics certifications are preferred.
• Three (3) months of related work experience is required.
• Must possess and demonstrate highly effective interpersonal, customer service and communication skills.

Must be fully vaccinated and boostered against seasonal Influenza and COVID 19 and provide required vaccination documentation, unless approved for an exemption or deferral by Southcoast prior to starting employment.
