- Provides direct-care services to patients including individual, group, and family sessions. Serves as a primary therapist to assigned patients and their families.
- Interviews patients and family members to develop a complete psychosocial assessment. Obtains information about patient history, health, and level of functioning.
- Serves as a member of the multidisciplinary team. Participates in the planning, implementation, and evaluation of treatment plans and updates.
- Works with patients and families to prepare solid aftercare plans, including referrals to community agencies and other professionals as needed.
- Complete and maintain accurate records of services provided and ensure documentation in the patient charts in a timely manner.
- Ensures program functions within requirements established by the Joint Commission and other state and federal agencies.
- Remains knowledgeable of current trends related to behavioral healthcare by participating in professional organizations, reviewing current literature, and communicating with representatives of other facilities.
- Responsible for creating and supporting a positive, professional, team-oriented, harassment-free work environment by understanding and complying with organization policies.
