Life Coach - Reports to Chief Behavioral Health Officer
Summary:
As an integral member of the care team, the Life Coach will have the opportunity to make a profound impact on the lives of people living with complex medical, behavioral health/substance use and/or chronic conditions. By ‘walking’ with the patient on their health journey and proactively addressing their needs, this role will help patients overcome barriers to accessing care and support them in achieving their health goals. In addition, this role will be on the cutting edge, working directly with Lynn Community Health Center to assist in the design and development of a new program intended to meet patients' needs between traditional patient visits. This position requires flexibility and may vary day-to-day to meet members where they are and improve the methods in which this is done. In addition, this position requires the ability to thrive in an ambiguous and ever-changing environment
Qualifications:
- Experience within the ACOs member population preferred
- Management or supervisory experience preferred
- Bachelor's degree preferred
Desired Skills:
- Bi-lingual (preferred)
- Must be flexible and adaptable to change.
- Must demonstrate the ability to work independently as well as within a broader team environment
- Must demonstrate excellent interpersonal communication skills
