Job Overview:
The Start of Care Nurse is responsible for the comprehensive in-person clinical assessment of the patient’s physical, functional, mental, psychosocial, and cognitive status in order to identify the needs of the patient and caregiver that will be addressed by Rvnahealth’s services.
The candidate selected will also be responsible for the admission of patients for service and determining and developing a plan of care. They will perform admission procedures as per RVNA policies using legible, accurate documentation and develop a plan of care consistent with the physician’s plan of treatment.

Job Requirements:
· A minimum of 2 years’ experience providing acute nursing care
· Home healthcare experience (1-2 years) is strongly preferred; those with SNF experience also desired.
· Knowledge of Medicare regulations strongly preferred but willing to train.
· CT Registered Nurse (RN) license is required.
· Strong physical assessment skills and computer skills are a must.

Location: New Milford, CT with an expectation of traveling to Ridgefield, CT if needed.

Pay: $80,938.00 - $88,000.00 per year
