ProHealth Partners, Inc. currently has Full time (Salary or PER visit full time) positions available in (*40 mile radius) within the Adrian Michigan service territory.*

Position offers a $7000.00 sign on Bonus. Must meet full time visit requirements and provide a 1 year commitment from date of hire.

$115.00 per visit for SOC Oasis, $75.00 Initial Evaluation, $70.00 Reassessment, $60.00-$65.00 for regular treatment visits, $60.00-$65.00 for D/C visits.

*Per visit rates are based on Home Health experience*

• 50 cents per mile (from first visit of the day through last visit of the day)

With our corporate office located in Perrysburg, Ohio, we currently service Northwest, Ohio, Southwest Ohio, the Greater Cleveland, Ohio area, as well as Southeast Michigan.

Job Responsibilities

• Communicating daily with office staff to accept new patients
• Complete paperwork and turn in payroll.
• Keeping open communication with patients and home health agencies to promote a positive and caring experience.
• Facilitating communication with Physical Therapist to manage patient care.
• Completion of Daily visit notes and complete daily and weekly paperwork.
• Attend training team meetings as needed.

Job Requirements

• Current Physical Therapist Licensure.
• Degree in Physical Therapy from an accredited school.
• Ability to travel to patient’s home or facility.
• Strong communication skills.
• Home healthcare experience required

Job Type: Full-time

Salary: $80,000.00 - $90,000.00 per year
