LICENSED VOCATIONAL NURSE - STAFF NURSE

Statements of Purpose

The Licensed Vocational Nurse Staff Nurse (LVN Staff Nurse) is responsible for the nursing assessment, medication monitoring and supervision of clients needs.

The LVN Staff Nurse reports directly to the Nurse Manager.

Major Task, Duties and Responsibilities

● Performance improvement data collection and reporting as directed by the Nurse Manager

● Medication monitoring process

● Accepting and transcribing phone orders from physicians & ensures physician sign off as per policy

● Nursing Assessment and ongoing assessment of client condition, including nursing notes each shift (restricted EHR access)

● Vital signs

● Maintenance of nursing supplies and over the counter medication supplies

● Nursing documentation audits (Med Room Audits, nursing note audit, etc.) ● Documentation and providing of shift report

● Medication counts as required

● Patient rounds 1 time per hour, dependent upon census/staffing

● Incidental Medical Services -including but not limited to

o Obtaining Medical Histories – Health Questionnaire, Nursing Assessment.

o Monitoring health status – direct observation, vital signs, CIWA, COWS, etc.

o Testing associated with detoxification from alcohol or drugs – UAs, BAC, Pregnancy Testing.

o Providing Alcoholism or drug abuse recovery services – TB Testing, Genetic testing, blood labs testing

o Overseeing client self-administered medications

o Treating substance use disorders, including detoxification – working with client's primary care physician or outside providers to coordinate care.

Skills, Knowledge and Competencies Required

● Knowledge of drug and alcohol dependency and mental health.

● Ability to properly document medication in the EHR.

● Ability to communicate effectively with physicians, supervisors, staff and clients. ● Knowledge of protocols for COWS and CIWAS.

● Effectively and thoroughly obtains medical histories from clients

● Ability to properly administer drug/pregnancy tests and BAC tests for clients

Prerequisite Qualifications

Following are the qualifications required to perform the essential functions of this position. Qualifications may be subject to modification based on the Americans with Disabilities Act.

_____ Must be at least 18 years of age

_____ Current Texas LVN License.

_____ One year experience required. Experience in a residential health care facility preferred.

_____ Class “C” Driver’s License (if driving for company), otherwise, will need Photo ID.

_____ CPR/First Aid certification required, obtain upon employment and maintain current thereafter. _____ Criminal Background Clearance required

_____ Health Screen required

_____ Negative TB Test Results required

_____ Develop computer skills adequate to perform word processing duties upon employment.

Vision, hearing, manual dexterity and hand-eye coordination must be adequate for performance of job duties. Able to sit at a desk, use keyboard, write and physically perform other job duties. Able to move about the facility to observe clients and staff.
