Please note: Pay is calculated per visit with higher pay rates for OASIS (start of care).

Locations: This position is advertised for Boston. We have offices in Jamaica Plain, Lawrence, Salem, Shrewsbury, Brockton and Fall River. We may have patients near your home.

We are flexible with hours that our RN want to work.

Position Summary:

• The available positions require nurses to effect quality and continuity of care on an ongoing basis. Our nurses ensure holistic and measured quality of care.
• Our nurses serve as a liaison for clinical communication, resource management and the promotion of quality and cost-effective interventions and outcomes. Case notes are important.
• Nurses help clients achieve wellness and autonomy through advocacy, assessment, planning, communication, education, resource management, service facilitation, and use of evidence-based guidelines or standards.
• Nurses provide emotional support and encouragement for patients.
• Must have experience working in home health care.
