QUALIFICATIONS: High School Diploma or equivalent

Education related to a field such as: psychology, social work, special education or related human service work preferred
At least one year residential experience
Basic knowledge of Microsoft Word and the internet
Valid Driver’s License

COMPENSATION: Competitive

JOB DUTIES:

• Provide operational duties assigned by the Program Manager.
• Write monthly progress notes, Individual Support Plans, assessments and objectives for annual reviews, etc.
• Attend and participate in all meetings and chart data collected from programs regarding individuals(s)
• Oversee shift documentations provided by staff.
• Assist with operational duties which may include: order of medication, supplies, banking, grocery shopping, and training new staff
• Assist with development of behavior plans or other interventions strategies.
• Instruct, supervise and evaluate individuals in all areas of community living as outlined in each support plan and record data on all instructional programs.
• Implement all behavioral plan interventions as assigned. These behavioral interventions may include physical restraint.
• Maintain valid Driver’s License and provide transportation as required by program.
• Assist individuals in scheduling and attending medical appointments and appropriately document scheduling and attending of social and leisure activities.
• Administer medications as required, in accordance with medication administration procedures.
• Actively assist and support individuals as needed with physical direct care.
• Produce reports, including daily shift reports, and other required documentation utilizing professional and appropriate process and skills
• Perform additional duties, or substitute for others as required
