- RN Case Managers provide direct patient care and services
- Provide Admissions, Home Visits and follow up
- Coordinate care with therapies for multidisciplinary approach
- Delegate care to LPNs and HHAs on your patient care team
- Facilitate patient and family independence in managing their comorbidities
- Promote wellness and goals and interventions to attain goals of care
- Electronic documentation
- Experience in Home Health Care, including Matrixcare a plus, 1-year minimum Nursing Experience Required
