Remarkable has a full-time opportunity available for a Transition Care Coordinator (Admissions Assistant) for our Carrollton facility.

Job Summary

Responsible for all incoming referral and admission inquiries. Provide a smooth transition for new patients into the facility. Assist in interviewing residents/guardian/sponsors for admission, and obtaining required admission information and signatures. Ensuring all documentation is uploaded into Remarkable EMR system, and updating EMR system of all daily referral/admission traffic. Position holder will be required to update and keep the daily census accurate. Drive the resident admission orientation program, new family orientation, prepare patient rooms for new and readmissions, handle transports, and be a point of contact for patients.

Educational Requirements and Experience

High school diploma or GED is required.

1 to 2 years of related experience and/or training; or equivalent combination of education and experience is preferred.

Computer savvy, especially in Microsoft Excel.

Must be able to multi-task in a busy office setting.
