FULL-TIME COMMUNITY BASED CASE MANAGER - BUTLER COUNTY, PA
Family Resource Specialist
We are seeking an experienced and compassionate Community Based Case Manager to join our team. In this role, you will support families in our community by providing them with resources, advice, and assistance. You will be responsible for connecting families with local services, such as housing assistance, healthcare, and other support services. As a Community Based Case Manager, you will also provide guidance and direction to parents and families in need, helping them find the resources they need to create a better future for their children. To provide excellent service, our families are our priority so working with their schedules is key to creating a healthy rapport with them. Evenings and/or weekends may be required to fulfill a 40-hour work week.

The qualified Community Based Case Manager will have:
- achelor's degree or high school diploma and experience working with children, youth, and/or families in a variety of settings or environments.
- Valid driver’s license, auto insurance and access to a vehicle.
- Pass current, applicable clearances.
- Pass pre-employment, post-offer drug screening.
- Ability to work non-traditional hours including evenings and weekends.

The qualified Community Based Case Manager will:
- Assess and engage families in service planning based on demonstrated needs.
- Plan and implement appropriate interventions using evidence based and best practice approaches.
- Assist families in accessing and navigating community resources.
- Attend court hearings and provide testimony as required.
- Transport clients utilizing personal vehicle, as necessary.
