AFC House Manager Job Description. Join our Adult Foster Care Team in providing hands on person-centered support to people with disabilities and mental health issues. The AFC House Manager will provide support to individuals by assisting them with their daily living needs in order for them to gain independence and lead fulfilling lives in the community. House managers oversee and manage the employees assigned to their site and are responsible for the overall management of the household.

AFC House Manager Responsibilities:

-Reviews all resident plans/protocols with staff.

-Communicates via phone/email and works alongside guardians, case managers, families and other team members.

-Composes and sends a weekly communication report to the resident's team.

-Ensures the resident's home and vehicle are clean, orderly and stocked with the necessary supplies.

-Ensures medications are being properly dispensed and documented on the Medication Administration Record.

-Performs monthly medication administration reviews and notifies the registered nurse/manager of any concerns.

-Attends and completes all required trainings including facilitating staff and resident team meetings.

-Participates in community activities with individuals

-Assists with self-cares such as bathing and dressing

-Grocery shops and assists the resident with meal planning, preparation.

-Monitors health care needs of the individual and communicates directly with the registered nurse

-Documents detailed narratives and behavior data daily

-Monitors and maintains safety of individuals providing direct care

-Follows programming outlined in the Individual Abuse Prevention Plan (IAPP), and Coordinated Services and Support Plan and Addendum (CSSP)

-Models behavior and mentors individual to increase their daily living skills

-Communicates and works alongside guardians, case managers, families and other team members

-Collaborates with co-workers in a team approach to provide quality care to individuals

-The ability to de-escalate crisis situations and behaviors that may require physical intervention on occasion

Qualifications:

-Two years experience serving people with developmental disabilities.

-Must be at least 18 years of age

-Must possess a valid driver's license (required) and must have a clear driving record.

-Ability to meet DHS standards on criminal background study

-Basic domestic skills (cooking, cleaning, laundry).

-Excellent verbal and written interpersonal communication skills.

Hours:
