Job description

We are looking for an experienced Hospice Intake LVN for our hospice business located in Santa Fe Springs. We are an established hospice agency servicing clients all over LA, Riverside, San Bernardino and Orange County. This is an exciting position as the new Hospice LVN will take on a crucial role and represent the company. Preference for Spanish bi-lingual candidates.

Responsibilities

• Explains the hospice program and eligibility of benefits to patients, family members, and other referral sources in an accurate, complete and thorough manner.
• Participates in coordination of hospice services and assigning staff members based on identified needs.
• Provides accurate and timely documentation of all consents to the Admission department.
• Evaluates need for medical supplies and equipment required for patient's home care. Orders and arranges delivery for patient post-hospital equipment and supply needs.
• Support quality assurance, quality improvement and pre-billing tasks.
• Participates in the on-call rotation and weekends.

Skills & Requisites

• Minimum 1-2 years of hospice experience ideal
• Licensed Vocational Nurse licensed by the California Board of Vocational Nurse
• Have excellent customer service skills
• Current CPR, health clearance, TB clearance.
• Valid Driver's License and Automotive Insurance required
• Valid CPR certification (American Heart Association)
• Must be able to collaborate with others and work independently
• Must demonstrate strong verbal skills and punctuality / attendance.
• Must have computer proficiency, and a working knowledge of Microsoft Office programs
• Bilingual in Spanish and English preferred

This position is mostly office based but could require occasional field visits in OC and LA.

Job Type: Full-time

Pay: $25.00 - $40.00 per hour
