Wraparound and REACH Supervisor
Wraparound Program

Position Summary: Working under the Wraparound system of care, which includes a unique set of values that emphasizes individualized, strength-based, trauma-informed, family-driven, community-based, and culturally sensitive care, provide supervision for program staff and manage the provision of all Wraparound and REACH program services.


Essential Functions:

1. Be a champion of the Wraparound process while providing supervision, leadership, and role modeling a strength-based environment to a team of approximately 8 Wraparound and REACH care coordinators and 1 lead care coordinator.

2. Provide weekly, individual, directive supervision, and coaching with each care coordinator for a minimum of one hour. Provide ongoing coaching and support regarding care coordinator’s strengths, areas of need, and potential strategies for improvement through the consistent use of Facilitator Passports/3 observations per month and supervision.

3. Support care coordinators in facilitating youth/family independence through a sustainable transition plan starting from beginning of enrollment. Review all pending disenrollments with Care Coordinators prior to submission of the disenrollment documentation to ensure for smooth termination of services.

4. Provide care coordinators with ongoing assistance in day-to-day service delivery, as well as crisis situations. Direct them to utilize Child and Family Teams for direction and assistance as much as possible.

5. Facilitate weekly agency care coordinator team meetings to share information regarding Wraparound Milwaukee monthly reports, updates, and areas in need of attention.

6. Provide/arrange for orientation and training in the Wraparound process to all new care coordinators, promoting individualized care and the use of natural/informal supports in care coordinators’ first 6 months of employment.

7. Ensure the provision of tools to maintain staff safety (i.e., cell phones, pagers, in-services on community safety) and remain receptive to care coordinators’ needs around community safety.

8. Review, assure accuracy, and timely submission of all enrollee consent forms, change of placement notices, Service Authorization Requests (SARs), court letters, progress notes, evaluation tools, POCs, Crisis Plans, provider referrals, and all other documents requiring Supervisor's signature/approval. Assure that accurate family demographic information in maintained in Synthesis.

9. Monitor quality provision of all services, including contract compliance and Agency Performance Reports(APRs) through report tracking in Synthesis. Engage in creative planning to help enhance care coordinator/Team performance.

10. Attend all initial visits made by care coordinators for all new youth/families to ensure appropriate/accurate explanation of the Wraparound process and programming.

11. Notify Wraparound Administration via email immediately when a new Care Coordinator has been hired, resigns, has a change in name or phone, cell and/or pager Numbers, and when any care coordinator family assignment changes are made.

12. Ensure the maintenance of a 24-hour on-call system to assure access to Wraparound Care Coordinators.

13. Facilitate and/or ensure planning and facilitation of monthly family activities, and submit evidence of activities to Wraparound Milwaukee Administration.

14. Attend and participate in bi-weekly Supervisor meetings with the Wraparound Milwaukee Management Team. Provide agenda items to promote discussion of current coaching needs.

15. Keep Wraparound Milwaukee informed of emergency or critical incidents.

16. Provide or arrange for coverage for care coordinators on vacation or absent, as needed.

17. Attend all trainings and in-services and assure that care coordinators do the same. Assist in coordinating of monthly topics.

18. Monitor SARs to assure that the Team has clearly defined in the POC the need for the requested service and have made fiscally sound decisions regarding the provision of services.

19. Run at least one team meeting once per year to maintain skill set.

20. Engage in quality assurance and quality improvement activities as identified by Wisconsin Community Services and Wraparound Milwaukee.


Other Duties and Responsibilities:

  • Participate in staffings, weekly/monthly meetings, and consultations, as needed.
  • Other job-related duties as may be necessary to carry out the responsibilities of the position.



Job Qualifications:

Minimal Qualifications:
Must possess a bachelor’s degree in a relevant area of education or human services and a minimum of three years of Wraparound care coordination or in-home experience(one year of which must be in Wraparound Milwaukee program), or possess a Master’s Degree in social work, human services, or a related field and have one year of Wraparound care coordination experience.

Working knowledge of computer applications preferred (Windows and Microsoft Word).

Ability to display cultural competence by responding respectfully and effectively to people of all cultures, languages, classes, races, ethnic backgrounds, religions, and other diversity factors in a manner that recognizes, affirms, and values the worth of each individual.

Work Relationship and Scope:
Reports directly to the Wraparound Program Manager. Has contact with a wide variety of individuals including children and family members, other program staff, including consulting Psychologist/Psychiatrist and other collateral contacts, neighbors, funders, Wraparound Milwaukee Administration, Milwaukee County Department of Health and Human Services, Division of Milwaukee Child Protective Services(DMCPS), Milwaukee County Department of Youth & Family Services(DYFS), Court officials, Milwaukee Public Schools(MPS) staff and administrators, staff of youth serving agencies and the general public.

Follow agency and Code of Conduct and adhere to established policies and procedures of the agency Milwaukee County/Wraparound. Dress in a respectful manner (refer to Wraparound Policy #040 - Dress Code) and maintain a respectful demeanor and presentation at all times. Conduct self in an ethical manner; maintain professional and respectful relationships with program staff, other WCS staff, youth/families, and all external persons and agencies involved with service provision; sensitivity toward cultural, ethnic and disability issues; demonstrate commitment to agency values and mission.

Knowledge, Skills and Abilities:
Working knowledge of positive child/youth development; patience and understanding of traumatic life situations; the ability to interact with escalating youth and/or parents in a calm and professional manner; ability to follow oral and written instructions; ability to respond appropriately in crisis situations; proficient with Microsoft Word and Excel; perform basic computer functions; uses time efficiently and effectively; ability to multi-task; sensitivity towards cultural, ethnic and disability issues.

Working Conditions:
Work is performed primarily in a busy office environment managing caseloads of children, young adults, and families. Some of the work is done sitting at desk using a computer; requires outreach in the community and families’ homes; much of the outreach is done in urban neighborhoods and several hours per day may be spent driving; hours average 40 per week; flexible work schedule include some hours outside the normal work schedule on evenings and weekends.


Physical Demands:
Duties require possible activity while monitoring or interacting with youth in the program. Staff may be required to use physical intervention in extreme situations. Driving required.
